우선 기존 주사제에서 탈피하려는 시도가 눈에 띈다. 대웅제약과 대웅테라퓨틱스의 비만 마이크로니들 패치는 최근 국내 임상 1상에 진입했다. 위고비, 오젬픽 등 주사제 대비 상대적 생체이용률을 확인하는 단계다. 마이크로니들 패치를 피부에 붙이면 세마글루타이드 성분으로 구성된 미세바늘이 녹아 약물을 피부 진피층으로 직접 전달한다. 주 1회 부착하면 된다. 주삿바늘을 무서워하는 환자도 쉽게 맞을 수 있는 것이 장점이다. 대원제약은 마이크로니들 패치 기업 라파스와 비만치료제 후보 물질 'DW-1022'를 공동 개발하고 있다. 위고비 주사제를 마이크로니들 패치 제형으로 바꾼 치료제다. 동아에스티도 마이크로니들 의약품 개발 기업 주빅과 유사한 제형의 비만치료제를 개발 중이다. 먹는 비만약 개발도 잇따르고 있다. 일동제약이 개발하는 'ID110521156'은 임상 1상 톱라인 결과에서 체중 감소, 혈당 강하 등 효과를 확인했다. 단계적 증량(MAD) 시험에서 200㎎ 투여군의 경우 평균 9.9%, 최대 13.8%의 체중 감량을 보였다. 케어젠의 글루카곤 유사 펩티드-1(GLP-1) 기반 경구형 체중 감량 펩타이드 '코글루타이드'도 임상 결과 효능과 안전성을 입증했다. 근육 손실 방지에 주력하는 기업도 있다. 한미약품이 개발하는 'HM17321'은 근육량을 늘리고 지방을 선택적으로 감량하는 효과를 내는 것으로 알려졌다. GLP-1을 비롯한 인크레틴 수용체가 아닌 CR F2 수용체를 선택적으로 타깃하는 우로코르틴(UCN) 2 유사체다. 위고비 등 GLP-1 치료제 최대 단점으로 지적받는 근육 감소 부작용을 극복할 수 있어 주목받는다. 한미약품은 최근 미국 식품의약국(FDA)에 이 치료제 임상 1상 진입을 위한 임상시험계획(IND)을 신청했다. 2031년을 상용화 목표 시점으로 잡았다. 디앤디파마텍 역시 근육 손실을 막으면서 지방을 줄이는 펩타이드 신약 개발을 연구하고 있다. 비만치료제 경쟁력 극대화를 위해 기업 인수·합병(M&A)이 단행될지도 주목된다. 화이자는 지난달 비만치료제 개발업체 멧세라를 인수한다고 밝혔다. 인수 규모는 최대 73억달러(약 10조2980억원)다. 화이자는 자체 개발하던 비만치료제 '다누글리프론' 임상이 실패로 끝나자 M&A 방식의 비만치료제 시장 진입을 추진해왔다. 송윤섭 기자 sys@etnews.com
