Black adults with type 2 diabetes have a higher risk for developing chronic kidney disease and heart failure than white adults, and the time from diabetes diagnosis to CKD or heart failure was 12 years or less in all age groups. In findings published in Diabetes Care from more than 1.5 million U.S. residents with type 2 diabetes, risks for developing CKD and heart failure varied by ethnicity, age group and sex. Additionally, most age groups with type 2 diabetes developed CKD and heart failure within a decade of type 2 diabetes diagnosis, and CKD had a greater mediation effect on the risk difference in heart failure compared with the mediation effects of heart failure on CKD risk differences between Black and white adults. Black adults with type 2 diabetes aged 18 to 59 years have a higher risk for heart failure than white adults. Data were derived from Paul SK, et al. Diabetes Care. 2022;doi:10.2337/dc21-2349. “We believe that the large cohort size, robust study design, use of advanced data mining methods, and appropriate assessment of confounders ensure reliable population-level estimates and reflect real-world patterns of CKD and heart failure risk in people with incident type 2 diabetes in a detailed evaluation by age, ethnicity and sex,” Sanjoy K. Paul, PhD, professor of clinical epidemiology and biostatistics at Melbourne EpiCentre, University of Melbourne and Melbourne Health, Australia, and colleagues wrote. “Our findings relative to different risk paradigms in African Americans and white Americans, including a mediation effect difference of heart failure and CKD, reflect the inherent differences in how the pathway of development of cardiometabolic diseases differ in these ethnic groups.”
Researchers analyzed deidentified patient-level data from the Centricity electronic medical record database. White adults (n = 1,491,672) and Black adults (n = 311,333) aged 18 to 70 years diagnosed with type 2 diabetes from 2000 to September 2018 were included. Participants were stratified into four groups based on age at type 2 diabetes onset: 18 to 39 years, 40 to 49 years, 50 to 59 years and 60 to 70 years. The presence of CKD and heart failure were confirmed through relevant disease identification codes. Researchers assessed the risks, mediation effects and time to diagnosis of CKD and heart failure for both Black and white adults as well as for each age group. Higher risks for Black adults
The risk for developing CKD among adults with type 2 diabetes was higher for Black adults compared with white adults aged 18 to 39 years (HR = 1.21; 95% CI, 1.17-1.26), 40 to 49 years (HR = 1.25; 95% CI, 1.22-1.28), 50 to 59 years (HR = 1.21; 95% CI, 1.19-1.24) and 60 to 70 years (HR = 1.1; 95% CI, 1.08-1.12). Black adults with type 2 diabetes also had a higher risk for heart failure than white adults aged 18 to 39 years (HR = 2.21; 95% CI, 1.98-2.45), 40 to 49 years (HR = 1.86; 95% CI, 1.75-1.97) and 50 to 59 years (HR = 1.54; 95% CI, 1.48-1.6). No significant difference in heart failure risk was found for adults aged 60 to 70 years. After stratifying further by sex, Black men aged 18 to 59 years had an 11% to 49% higher risk for developing CKD than women. White men aged 18 to 39 years had a 23% higher risk for CKD than women, whereas white men aged 50 years and older had a 7% to 14% lower CKD risk than women (P < .01 for all). The mean time from diabetes diagnosis to first CKD event was between 5.1 and 9.7 years across all age groups in Black and white adults with type 2 diabetes. The time from diabetes diagnosis to CKD was 3.6 years longer for Black adults aged 18 to 39 years compared with those aged 60 to 70 years. White adults aged 18 to 39 years had a time to CKD event 4 years longer than those aged 60 to 70 years. The mean time from diabetes diagnosis to a heart failure event was between 6.9 and 12.1 years across all age groups. The time to a heart failure event was 3.4 years longer in Black adults aged 18 to 39 years compared with those aged 60 to 70 years, and 4.1 years longer in white adults aged 18 to 39 years compared with those aged 60 to 70 years. The overall independent mediation effect of CKD in the context of risk difference in heart failure between Black adults and white adults was 54% in the 18-to-39-year age group, 40% in the 40-to-49-year age group, 41% in the 50-to-59-year age group and 91% in the 60-to-70-year age group. The mediation effect of heart failure in the context of risk difference in CKD between the two ethnicities was 13% in adults aged 18 to 39 years, 39% in those aged 40 to 49 years, 35% in adults aged 50 to 59 years and 15% in those aged 60 to 70 years. “Both CKD and heart failure were apparent in some individuals within a few years of a type 2 diabetes diagnosis, even among those diagnosed at age < 40 years and particularly within the African American population,” the researchers wrote. “An urgent need exists to incorporate multidisciplinary care in the identification of high-risk patients from type 2 diabetes onset, along with legislative support promoting equitable access to therapies and care, especially for young, vulnerable and underrepresented patient populations.”

Eli Lil­ly was able to fend off gener­ic com­pe­ti­tion to its chemother­a­py drug Al­im­ta for quite some time, de­fend­ing its patents in a se­ries of le­gal bat­tles. But as of last week, the drug has 14 new copy­cats to con­tend with. The FDA last Wednes­day ap­proved 14 ab­bre­vi­at­ed new drug ap­pli­ca­tions (AN­DAs) for Al­im­ta gener­ics, in­clud­ing ones from Pfiz­er’s Hos­pi­ra and Dr. Red­dy’s, which had pre­vi­ous­ly en­gaged in patent bat­tles with Eli Lil­ly. All 14 ap­provals were for non-squa­mous non-small cell lung can­cer (NSCLC) and mesothe­lioma, the lat­ter of which Al­im­ta was first ap­proved to treat in 2004. Since then, the drug has seen sev­er­al la­bel ex­pan­sions, in­clud­ing in com­bi­na­tion with Mer­ck’s cash cow Keytru­da. In its prime, Al­im­ta was rak­ing in up­wards of $2.7 bil­lion — but last year, sales de­clined 12% to rough­ly $2 bil­lion. Out­side the US, sales fell 22% be­cause of gener­ic com­pe­ti­tion in cer­tain mar­kets. In its Q4 re­sults, the phar­ma gi­ant ad­mit­ted that sales will like­ly slip fur­ther as gener­ics hit the US mar­ket for the first time this year. In ad­di­tion to Hos­pi­ra and Dr. Red­dy’s, 10 oth­er com­pa­nies won gener­ic ap­provals last week, in­clud­ing: Zy­dus Phar­ma­ceu­ti­cals, Eu­gia Phar­ma, Wa­ver­ley Phar­ma, Bio­con, Apo­tex (which snagged three ap­provals for dif­fer­ent dose for­mu­la­tions), Jiang­su Han­soh Phar­ma­ceu­ti­cal, Nang Kuang Phar­ma­ceu­ti­cal, Qilu Phar­ma­ceu­ti­cal, Ac­cord Health­care, and Fre­se­nius Kabi USA. While Al­im­ta is los­ing its foothold in the NSCLC space, Eli Lil­ly scored an ap­proval in the first-line set­ting for its oth­er can­cer drug, Cyra­mza, back in 2020 in com­bi­na­tion with Roche’s Tarce­va. Cyra­mza sales have been flat for the last cou­ple of years, to­tal­ing just over $1 bil­lion dol­lars in 2021. There’s al­so Retev­mo, the ki­nase in­hibitor Lil­ly snagged in its $8 bil­lion buy­out of Loxo On­col­o­gy, which crossed the FDA fin­ish line in 2020 for cer­tain RET-dri­ven lung and thy­roid can­cers. That drug made near­ly $115 mil­lion last year. Eli Lil­ly has been es­pe­cial­ly pro­tec­tive over Al­im­ta. Back in 2019, a fed­er­al ap­peals court ruled in the phar­ma gi­ant’s fa­vor, de­cid­ing that the Hos­pi­ra and Dr. Red­dy’s ver­sions would in­fringe on its patents. De­spite fight­ing to keep copy­cats off the mar­ket, Eli Lil­ly saw its first gener­ic com­pe­ti­tion back in Feb­ru­ary, when Ea­gle Phar­ma­ceu­ti­cals launched its Pem­fexy. My­lan (which com­bined with Pfiz­er’s Up­john unit to form Vi­a­tris) al­so se­cured ten­ta­tive ap­proval for its Al­im­ta gener­ic back in 2019 but was pre­clud­ed from mar­ket­ing due to le­gal pro­tec­tions. Just when the pharma industry thought today’s $3 billion GSK buyout might kick off M&A season, the Department of Justice and the Federal Trade Commission said later in the afternoon they’re putting on a 2-day workshop in two weeks that will focus on “new approaches to enforcing the antitrust laws in the pharmaceutical industry.”
The virtual workshop will include a keynote from FTC Commissioner Rebecca Kelly Slaughter, who has previously sought to block key pharma mergers and recently pulled together international partners from Canada, the UK and the EU, to identify concrete and actionable steps to review and update their analyses of pharma mergers. This workshop is the culmination of the work of this group of regulators, known collectively as the Multilateral Pharmaceutical Merger Task Force, the FTC said. Read More

The prospect of the Centers for Medicare & Medicaid Services (CMS) paying for Biogen’s controversial Alzheimer’s drug Aduhelm opened up a multi-billion-dollar can of worms late last year that CMS now says it won’t right until 2023, leaving the agency with little explanation for major premium and deductible increases for seniors this year. Back in November 2021, CMS explained that Medicare Part B will have to increase its standard monthly premium — from $148.50 in 2021 to $170.10 in 2022 — in part because of the massive spending that could occur should the agency sign off on a national coverage decision for Aduhelm, and its $56,000 annual price tag next year. Another 14 generics threaten Eli Lilly's blockbuster chemotherapy Alimta

The global anesthetics market is expected to grow from $9.24 billion in 2021 to $9.88 billion in 2022 at a compound annual growth rate (CAGR) of 7.0 %. The market is expected to reach $12.24 billion in 2026 at a CAGR of 5.5%. Major players in the anesthetics market are Baxter, Abbott Laboratories, AstraZeneca, F. Hoffmann-La Roche AG, B. Braun Melsungen AG, AbbVie Inc., Hospira, Aspen, Boehringer Ingelheim International GmbH and Eisai Co. Ltd. Japanese drug major Astellas Pharma has confirmed the US District Court for the District of Delaware ruling that all asserted claims of the 8,106,183; 8, 524,883; and RE47301 patents for Lexiscan (regadenoson injection) 0.4mg/mL are not infringed. The patent infringement trial involved one Abbreviated New Drug Application (ANDA) filer, Hospira, which is requesting permission from Food and Drug Administration to manufacture and commercialize a generic Lexiscan product before expiration of the Lexiscan patents. Astellas plans to appeal the decision and to take legal action to prevent Hospira from launching generic Lexiscan pending resolution of the appeal. Astellas is reviewing the potential financial impacts of this district court decision for the fiscal year ending March 31, 2023.

A judge in Delaware has ruled against Astel­las Phar­ma and Gilead in a long-run­ning patent case over Pfiz­er-on­wed Hos­pi­ra’s gener­ic ver­sion of Lex­is­can. The case kicked off in 2018, af­ter Hos­pi­ra sub­mit­ted an Ab­bre­vi­at­ed New Drug Ap­pli­ca­tion (AN­DA) for ap­proval to mar­ket a gener­ic ver­sion of Gilead’s Lex­is­can. The drug is used in my­ocar­dial per­fu­sion imag­ing (MPI), a type of nu­clear stress test. Gilead and Astel­las al­leged that Hos­pi­ra’s AN­DA sub­mis­sion for re­gadeno­son in­fringed up­on sev­er­al patents, and sought a de­clara­to­ry judg­ment that Hos­pi­ra’s man­u­fac­tur­ing of the prod­uct would con­sti­tute a di­rect and in­duced in­fringe­ment. Hos­pi­ra de­nied the al­le­ga­tions, as­sert­ing in its de­fense that the patents are in­valid and fil­ing coun­ter­claims seek­ing judg­ments of non­in­fringe­ment. Af­ter a three-day bench tri­al, Judge Colm Con­nol­ly ruled against Gilead and Astel­las. Con­nol­ly stat­ed that Gilead and Astel­las failed to prove any di­rect in­fringe­ment, and did not meet any bur­den to es­tab­lish that in­fringe­ment was breached. The par­ties are await­ing a fi­nal judge­ment. The de­ci­sion comes as Astel­las wrote off more than half a bil­lion dol­lars in loss­es last month and gut­ted can­di­dates that were once thought to have mas­sive po­ten­tial. The com­pa­ny booked an “im­pair­ment loss” over two fis­cal quar­ters back in April, about a year af­ter writ­ing off $540 mil­lion fol­low­ing a clin­i­cal hold on its lead gene ther­a­py AT132. For Gilead , while the com­pa­ny re­mains high on the For­tune 500 list , it suf­fered a set­back ear­li­er this year when the FDA slapped par­tial holds on stud­ies eval­u­at­ing ma­grolimab with azac­i­ti­dine. The FDA lift­ed its par­tial hold on those stud­ies last month, and Gilead said it’s on track for an in­ter­im read­out in 2023. AUTHOR

The Union for International Cancer Control (UICC) has gathered a slate of Big Pharmas for its new collaboration in hopes of increasing access to cancer medicines in lower income countries, UICC announced yesterday. Dubbed ATOM, or Access to Oncology Medicines, the coalition includes AstraZeneca, BeiGene, Novartis, Bristol Myers Squibb, Roche, Gilead, and Sanofi, among other organizations. The goal of the partnership is to increase generic and biosimilar development of cancer drugs as well as license out essential medicines to these countries. As the cases of monkeypox now sit at well over 100 worldwide and have spread to multiple continents, the orders for any type of vaccine against monkeypox are seeing nations and medical bodies looking to get their hands on anything and everything. And now SIGA Technologies seems to be getting in on the action. According to Euronews , SIGA Technologies, a pharmaceutical company that is focused on providing medical countermeasures to biological and chemical attacks, is now in talks with several European authorities looking to stockpile its antiviral that can counter monkeypox. The drug known as tecovirimat or Tpoxx was approved by the FDA in 2018 as a vaccine for smallpox but was approved by the European Medicines Agency to also act against monkeypox, cowpox and complications from immunization with vaccinia. The FDA on Monday said it updated its recommendations for the clinical development of new antibacterial drugs, and thanks to a new clutch of drugs, sponsors will have to conduct noninferiority trials that include subjects with infections caused by certain drug-resistant organisms because an effective active control can be provided. The guidance, which is likely to be released within the next day or so, builds on a 2017 document and specifies what noninferiority trial designs may be used “with a wider NI margin,” including cases in which the trial population is enriched for subjects with infections caused by certain drug-resistant organisms, FDA said.
