Dr. Schroer has one of the lowest patient readmission rates post-surgery in the country. The program automatically sends patients texts with educational information, tips and reminders for their self-care once home. Most of the texts are questions requiring patient response, that patient self-reported data allows the program to automatically red flag at-risk patients, sending real-time alerts to care management teams for immediate patient intervention. SanusEO offers a Transition Care Patient Engagement Program for Diabetes, Congestive Heart Failure and now Joint Replacement. Programs for COPD and Gestational Diabetes will be released in May, 2015. SanusEO’s Transition Care Program is a proven product with a 57% reduction in 30-day readmissions among diabetes patients at a Midwest hospital.
